A Second Opinion

Dr. Sams has evaluated thousands of patients for surgical candidacy, providing a second opinion about surgery, injections, neuroablations, spinal cord stimulators, morphine pumps, etc. If you have been recommended for surgery or another type of major intervention, and are uncertain about how to best proceed, schedule a 30 minute consult with Dr. Sams and let him assist you in making your decision, as he has helped thousands of others make the same decisions.

You and Dr. Sams will discuss: the procedure itself; the risks and benefits of the procedure; realistic expectations for the procedure outcome; what your post procedure course will be like; additional information you may need to make a fully informed decision; the results of the PPP Assessment; and how to gain support from family and friends for your procedure.  

You will complete a brief, intake form that provides Dr. Sams with a snapshot of your history. Fax relevant diagnostic reports (MRI, X-Ray, CT myelogram, Discogram, EMG, Nerve Conduction Study, PET, and Ultrasound), physician reports, or other information to him at 877-545-7272. For the most effective consultation with Dr. Sams, complete the PPP Pain Assessment and review the summary scale results.
